What should I do before I visit?

  • If possible, Please download, fill-out, and print or email all documentation. If not, this will be available to you at our office. 

  • Collect key past medical history, previous treatments, current medications, and other important information.

  • List all signs and symptoms that may be of importance to your therapist.

  • Make a list of questions that you would like to discuss with your therapist on your first visit.

  • Please call our office to verify your insurance benefits.

  • Please arrive 15 minutes early for your first appointment.

  • Bring a copy of your insurance card and a photo ID.

Insurance & Payment Options

We gladly accept the insurance plans listed below.  If your plan is not listed, please call us at (910) 490-1269 as we are always adding new insurance plans.  On a case-by-case basis, insurances may approve out-of-network care comparable to in-network benefits.  We can help you navigate that request.  As a courtesy to our patients, we will verify your health benefits prior to your arrival to determine if there will be a copay, deductible, or coinsurance for therapy services.  Please understand that any information we relay on behalf of your insurance is simply a courtesy and is not a guarantee of payment.  We always encourage our customers to confirm all benefits and eligibility information.

*If you do not see your insurance above and would like to be seen in our clinic, please give us a call to discuss.  This list is being updated regularly and it is not uncommon for an insurance company to make exceptions and allow an individual to secure care with an out-of-network provider.

For those patients who do not have health insurance or those who have maxed out their benefits, we offer a private pay plan (Self-Pay).  Please contact THRIVE Physical Therapy today for more information on our fees for service and payment details and please ask about ‘session bundles’ that are available in our office. 

What can I expect from my first visit?

  • You will register with our friendly front office staff. They will help you navigate the insurance process and treatment program so you can focus on your recovery.

  • You will then meet with your physical therapist who will ask questions about your medical history, current pain, or injury so that we can fully understand your problem.

  • Your movement, strength, coordination, and more will be evaluated.

  • Your therapist will recommend and prepare a treatment plan for optimal results.

  • There may be some minor treatment performed the first day to get you started on your road to recovery.

  • You will schedule out your full Plan of Care with the front desk after the appointment. We offer many convenient appointment times, however, prime times can become busy, so please bring your personal calendar and book ahead as much as possible.

  • After your evaluation, the results of the evaluation will be submitted to your referring physician or other medical professionals.  Depending on your insurance, the results may also be submitted to your insurance company for authorization of your visits. 

  • Then we’ll get into the full swing of the treatment plan during your next visit!
